ভোলা সদরের কালীবাড়ি রোডের শ্রী শ্রী কেন্দ্রীয় কালী মাতা মন্দিরটি আনুমানিক দ্বাদশ শতকের মাঝামাঝি থেকে শেষের দিকে (১৯৫০-১৯৭০ দশকের মধ্যে) ভোলা শহরের বিকাশ এবং স্থানীয় সনাতন ধর্মীয় সম্প্রদায়ের প্রসারের সাথে সাথে প্রতিষ্ঠিত হয়েছিল বলে স্থানীয়দের সূত্রে ধারণা করা হয়। ভোলার মূল শহর ও কালীবাড়ি রোড এলাকাটি যখন ধীরে ধীরে গড়ে উঠতে শুরু করে, তখনই স্থানীয় ধর্মপ্রাণ ভক্তদের উদ্যোগে মাতৃপূজা ও অন্যান্য উৎসব আয়োজনের জন্য এই মন্দিরটি স্থাপন করা হয়। তবে কোনো শিলালিপি বা লিখিত নথি ডিজিটাল আর্কাইভে না থাকায় এর সুনির্দিষ্ট প্রতিষ্ঠাকাল বা তারিখ স্পষ্টভাবে লিপিবদ্ধ পাওয়া যায় না।
